Just to note, Paul Graham's statement is "not consistently candid." Telling someone to stop screwing around with other things and do his job or resign IS firing him! At the c-suite level they are often allowed to resign in order to save face for everyone involved but it's still a firing. The fact that no one wants to admit that it's a firing doesn't suddenly change what it is. A duck is a duck, even if you call it an aquatic fowl instead.

Not sure, in my book a "firing" is when you're not given a choice to stay or you can only stay under very demanding conditions. Altman was offered a clear choice and the condition ("please don't be the CEO of another company") was extremely easy to implement.

I did see that before publishing. Not accepting claims is not a denial. I do not accept that people find it acceptable to put certain toppings on pizza. They still do it.
